Reablement
Express InterestTender Description
Reablement is a short-term service, typically lasting up to 4 weeks, but longer by exception, focused on supporting people to regain confidence, skills and independence. Currently around 70% of people in Camden who draw on reablement do so following a hospital stay, while others are supported to improve independence whilst still at home and in their community. This is a service specification for the provision of reablement delivered in the home for Camden residents aged 18 years or over who would benefit from reablement. It is an outcomes-focused specification: it focuses on outcomes rather than tasks; it offers greater scope and flexibility for Providers to organise reablement with an individual resident in a range of ways to achieve their required goals through reabling, relationship-based and trauma informed support. It describes the delivery model and the key service outcomes and should be read in conjunction with the terms and conditions of the contract. Reablement services will operate in three localities in Camden (North West, North East and South). One Provider will operate in each locality. Lot 1: North West Locality (Camden wards: Fortune Green, West Hampstead, Kilburn, South Hampstead, Hampstead Town, Frognal and Belsize). Lot 2: North East Locality (Camden wards: Gospel Oak, Haverstock, Kentish Town North, Kentish Town South, Camden Square, Highgate). Lot 3: South Locality (Camden wards: Primrose Hill, Camden Town, Regents Park, Kings Cross, Bloomsbury, Holborn and Covent Garden, St Pancras and Somers Town)
