The Service will be available for adults aged 18 or over who have a disability, and have been assessed as eligible for a service from Adult Social care (‘disability’ means a person aged 18 and over who is resident within the City of York who has been assessed by Adult Social Care as having a physical or mental impairment that has a ‘substantial’ and ‘long-term’ negative effect on their ability to do normal daily activities) The Service will provide care and/or support for customers of the Council’s Adults Services who have a disability. The services defined below may be delivered by one Provider or a variety of Providers to form the Customer’s package of support.
