Independent Living Scheme (ILS)

The scheme provides temporary accommodation for assessment and rehabilitation for patients leaving hospital, thereby supporting stable housing arrangements and improved flow from hospital and reducing hospital readmission rates. The service consists of four flats borough-wide, offering up to six weeks’ accommodation for those unable to return home directly from hospital. This supports also offers comprehensive long-term needs assessment and helps address issues such as housing, debt, and health. We are looking for a Provider to deliver this service.
