The Isle of Wight Council (“the Council”) is establishing an approved provider list for the delivery of NHS Health Check services on the Isle of Wight. The NHS Health Check programme aims to prevent heart disease, stroke, type 2 diabetes and kidney disease, and raise awareness of dementia both across the population and within high risk and vulnerable groups within the age range 40 – 74. Potential providers wishing to be admitted on to the approved provider list must complete the Enrolment Questionnaire (“EQ”) and be able to demonstrate that they meet all of the minimum criteria listed. Any provider that is able to demonstrate that they meet this criteria will be admitted on to the approved provider list and will be able to carry out NHS Health Check services and recharge the Authority for the activity undertaken. The approved provider list will be established and will go live on the 16 January 2020 and will be used for all NHS Health Check activity for the duration of the approved provider list. The approved provider list will operate until 31 March 2026, unless terminated earlier in accordance with the termination provision of the Terms and Conditions.
